Trillium Health is an Accountable Care Organization (ACO) working to change the landscape of healthcare in Northern Michigan.

 

What is an Accountable Care Organization (ACO)?

An ACO is a group of physicians, hospitals and other healthcare providers who work together to provide Medicare Fee-For-Service beneficiaries with high-quality, coordinated care. The providers within an ACO take their shared responsibility one step further through efforts to reduce the rate of growth in overall healthcare spending. Communication is streamlined between patient and provider and care is coordinated among providers, which results in efficient, higher-quality, lower-cost care.

 

How will it affect me as a Medicare Beneficiary?

As a Medicare Beneficiary, you will see no change to the benefits you already receive.

•    Trillium Health is not an insurance company or an HMO, so you are free to see any doctor you choose.
•    There will be no change to the out-of-pocket costs associated with your healthcare.
•    Your prescription benefits will remain the same, including Medicare Part B.
•    Supplemental insurance plans will not be affected by Trillium Health

Rather, we hope you will notice you have providers who are more focused on you and improving your overall health.

•    Wellness, preventing illness and managing chronic conditions will be at the center of your care, so that treatments and/or hospital stays can be reduced.
•    Test results and information regarding your care will be shared among your providers more easily, which will eliminate the need for duplicate services and allow for more effective decisions about your care.
